Wellness Mentor/Health Coach- Lean MD at Sierra Pacific Orthopedics

Location: Fresno, CA

Sierra Pacific Orthopedics, the leading full-service orthopedic group in the San Joaquin Valley, is seeking a full-time Wellness Mentor/Health Coach to add to our Lean MD Practice.

JOB SUMMARY: Performs administrative and clinical duties in coordination with the Physician to provide quality health care to patients.

EDUCATIONAL REQUIREMENTS:

  • Coursework in health, medical nutrition, weight loss and/or physical fitness
  • Medical Assistant Certification is a plus

QUALIFICATIONS AND SKILLS:

  • Strong communication skills with staff, physicians and patients
  • Good analytical and problem-solving skills
  • Ability to prioritize work flow in a fast-paced team environment
  • Strong computer skills, Electronic Medical Records (EMR) experience, preferably with Nextgen
  • Excellent social media skills with the ability to generate new ideas on how to connect to patients through various social outlets

RESPONSIBILITIES INCLUDE, BUT ARE NOT LIMITED TO:

  • Rooming patients and taking vitals
  • Documenting patient information in EMR
  • Partner one-on-one with patients on their weight loss journey
  • Provide step by step counseling to keep them motivated
  • Ensure patients success on the program by conducting weekly consultations, making care calls, and live confirmation calls
  • Mentors create true patient partnerships by assisting them in identifying challenges, setting realistic goals, and achieving life-changing habits
  • Mentors thrive on working in a fast-paced team environment and helping people achieve a healthier lifestyle
  • Social media communications

TYPICAL PHYSICAL DEMANDS:

Position requires prolonged sitting, some bending, stooping, and stretching. Some patient assistance will be required. Good hand-eye coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone and other office equipment is also required. Employee must have normal range of hearing and eyesight to record, prepare, and communicate appropriate reports.

Medical office environment.
